The electronic purse can be purchased at the Customer Service Office at MZK BOK and at PDB Ticket Top-Up Points (NO NECESSARY TO COMPLETE THE APPLICATION).

It is possible to personalize the diskette (recording user data in the e-card's memory system) - issuing and collecting a personalized bearer card will be carried out in the same way as in the case of a personal card, based on an appropriate application.

In order for the card to be ready for use when 'purchasing', the e-wallet must be topped up with a minimum amount of PLN 10 to be used for travel.

The personal e-Card must be collected in person in order to verify the information provided - if the passenger cannot collect the electronic card on his own, he must authorize another person (authorization template can be downloaded here) - the authorized person should present his ID card

Top-up on delivery

Each e-Card will be issued with an appropriate top-up - an electronic purse with a minimum amount of PLN 10, a season ticket with an amount sufficient to purchase any season e-ticket.

HOW TO COMPLETE THE APPLICATION

    Available types:

    • name with photo - if you are applying for an e-card for the first time
    • duplicate e-Card - if you have lost or destroyed your e-card and want to receive a duplicate floppy disk
    • data update - if you have an e-card but want to change your personal data in the system

    Personal data
    • fields with an asterisk are mandatory
    • By providing your phone number or e-mail address, you make it easier for us to contact you

    Ticket type
    • here you select which e-ticket you will top up when collecting the e-card - funds for an electronic purse or a season e-ticket
    • you can check more than one box because the e-card can contain two different season e-tickets (for different validity periods) and e-wallet funds up to PLN 250
    • if you are entitled to free travel, leave all fields blank

    Eligible for relief
    • If you are entitled to reduced fares, please indicate why here
    • attach a copy of the document entitling you to the discount to your application, e.g. ID card, document certifying age, or show the original upon delivery
    • In the DISCOUNT PERIOD (above the gray field) enter the expiry date of the abovementioned. document or select 'INTEREST'
    • If you are not entitled to reduced fares, leave all fields blank

    Eligible for free travel
    • If you are entitled to free travel, please indicate why here
    • attach a copy of the document confirming your rights to the application, e.g. ID card, large family card, document certifying age, etc. or show the original upon collection
    • in the field VALIDATION PERIOD OF FREE TRAVEL (above the gray field), enter the expiry date of the above-mentioned. document or select FOREVER
    • If you are not entitled to free travel, leave all fields blank

    Photo
    • the application submitted in person at BOK or by post should be accompanied by a photo with your name and surname on the back - the photo will be returned when collecting the e-card
    • the application submitted online must be accompanied by a photo in PDF or JPEG format signed with your name and surname (can be scanned)
    • the photo should show the entire face from the front on a light background
    • in the case of ticket control, the photo printed on the disk allows identification of the e-card owner, therefore you will not need to present an ID document

    Reason for issuing a duplicate e-Card

    You only fill in the form when applying for a duplicate e-card - if not, leave the fields blank

    Signatures
    • the submitted application should have three signatures (above the gray field) - the signature may be submitted earlier or only when submitting the application
    • the signature in the field at the very bottom will be made only when collecting the e-card
